MQ6 Fillable Forms

Consider using the Menopause Quick 6 (MQ6) menopause assessment tool during visits with your mature women.

The MQ6 tool focuses on questions that address the most common menopausal symptoms and those for which there are evidence-based menopause-specific treatment options. These six simple questions are inquiries recommended by guidelines around the world. The rationale and interpretation of these questions can be found in Table One of the CFP journal publication.  These questions are also designed to provide you with information that patients often do not volunteer, but which significantly affects their quality-of-life during the perimenopause and, for some women, well into the postmenopause.

Many women report that their healthcare professionals do not take the time to address their menopausal concerns! With this in mind, the MQ6 was written using lay language and open-ended questioning: designed as a discussion aid to help start the conversation between women and their healthcare professionals.

The MQ6 tool can be easily administered during clinical encounters and created into a stamp to enter into the electronic medical record. A second way to use this tool would be to use the online/printable PDF formatted MQ6 where the questions have been reworded to provide for yes/no answers (see button below).

The Menopause Quick Six Questionnaire: If a patient answers yes to question 1, they may be perimenopausal or menopausal and further clarification should be obtained.  A “yes” to questions 2, 3, or 4 may indicate symptoms amenable to treatment with Menopausal Hormone Therapy (MHT) or other therapies. A “yes” to questions 5 or 6, while not indications for MHT, do indicate relevant symptoms that should be considered when formulating your menopausal treatment plan.
